New: Howard House – Transition Service

At Howard House, we provide a specialist step-down service for adults with learning disabilities and/or autism who are ready to leave hospital but need a safe, supportive environment before moving into long-term accommodation.

We deliver individually tailored support, with a focus on building confidence, developing life skills, and supporting emotional wellbeing. Our experienced team works closely with local authorities and NHS commissioners across the UK to ensure transitions into community life are safe, timely, and fully compliant.

Supported Living through Consummate Care is about providing a secure, safe and stable home which enables individuals to retain choice and independence. We help vulnerable adults, including people with learning and or/physical disabilities, to live independently in the community.

Supported Living arrangements are very flexible and are designed to give each person choice and control over their home and the way they live their life.
